Reach out—first to your nephrologist, or directly to us—if any of these signs appear:
- Two or more unplanned hospital or ER visits in six months
- Creatinine around 8 mg/dL (or 6 mg/dL with diabetes) or an eGFR in single digits

- Dialysis is no longer tolerated—or a decision has been made to discontinue it
